Welcome to the EPC Benefit Resources, Inc., section of our website. We hope the information you find here will be helpful as you continue in or want to become enrolled in EPC Benefits for the 2018 calendar year.

Open Enrollment is from November 1-22, 2017. This is the timeframe when you (EPC churches and Benefits participants) must let us know if you want to come into our plans, maintain your current coverages for 2018, or make changes to your health care coverages for the new year. Here’s what needs to be done:

  • NOTHING, if you want to stay in the same Plan option for 2018.
  • COMPLETE ALL REQUESTED INFORMATION ON AN ENROLLMENT FORM if you want any changes in your benefits. The new form MUST be completed and submitted during open enrollment, November 1-22, 2017. Changes become effective January 1, 2018.

PREMIUM RATES 

 

The monthly premium rates for the 2018 Medical, Dental, Vision, Life and AD&D, and LTD plans can be found at www.epc.org/healthrate2018. For your convenience, Dental, Vision, Life and AD&D, and LTD plan rates are listed below the Medical Plan calculator. This way you have all insurance rates in one location.

There will be a slight increase in premiums. The new rates for the $50,000 Life and AD&D insurance is $13.25 per month for those under 65. This is a 6 percent increase, after no change in rates for the last three years. The LTD premium will be $0.28 per $100; an increase of $0.03. Previously, we offered two levels of LTD coverage, 60 percent to $2,000 and $4,000. This year we added a third level, $6,000. This is to ideally offer adequate coverage across all levels of income.

ENROLLMENT AND/OR CHANGE FORM 

 

You must complete all requested information on a new form if you want to make any changes to your benefits plan. These MUST be completed during the open enrollment period, November 1-22, 2017. Changes become effective January 1, 2018.

If you want to stay in the same Plan option, you do not need to complete a new form. Your existing 2017 Plan choices will carry over into 2018.

CHURCH BENEFITS ELECTION FORM 

 

Church Administrators: click here to download the form we require you to complete to indicate which benefits you will offer to your staff. The EPC Administration Office must have this completed form on file for employees from your church to enroll in the EPC Benefit Plans. Form can be faxed to 412-224-4465 or scanned and emailed to EPC@cdsamin.com.

TERM LIFE, AD&D, AND LTD 

 

New for 2018: The Life and AD&D policy will be bundled with the Long-Term Disability policy. For most participants, this is not issue. However, there are a few who have elected one or the other—this option will no longer be available.

EXPRESS SCRIPTS 

 

Express ScriptsNew for 2018: We are changing the delivery method of maintenance drugs for 2018. The BRI Board has enrolled in the Express Scripts Smart90 program, which allows for maintenance drugs to be delivered through mail order or for you to purchase at your local Walgreens. AMPLIFON 

 

AmplifonNew for 2018: Participants enrolled in the EPC benefit plans, including the Retirement Plan, will have access to the Amplifon hearing aid program. This program provides discounted hearing aids through more than 5,000 audiologists throughout the country. There are other benefits provided when working through this network. VOLUNTARY BENEFIT OPTIONS 

 

Colonial LifeTerm Life Insurance, Accident Insurance, and Short-Term Disability Insurance are available as optional benefits through Colonial Life.
